Here is a sample character sheet, which covers the basic fields a good character sheet bio should need. Please go ahead and change it as you wish. Add whatever that seems to be lacking, or don't bother filling out fields that don't apply to your character. Character sheets are meant to review your personal character, so make your character YOU.

Character Name: (Your character's name [which includes titles, rank, surname,etc.] to be called by other characters)

Character Class: (Character Class: Assassin, Monk, Warrior, Knight, or whatever your character 's role is during a RP)

Race: (Your character's race)

Gender: (Your character's gender [male or female?])

Age: (Your character's age, and if he/she doesn't have a known age, describe how old they look)

Faction: (The side your character is on, in terms of national allegiance, if any exist. If any doesn't exist, just call them Wanderer or something within same terms)

Physical Traits: (Give your character a sense of "form" or envisioned body. Includes shape, muscular properties, etc.

Mental Traits: (What are your character's morals, fears, thoughts? What does he/she hate? What do they like? What is their vision of the world?)

Biography: (Your character's bio, or in other terms, what was their life before they entered the RP?)
